Checklist for Applicants Seeking An Initial Teacher License
This page lists all required information you need to assemble before you begin the online application.
Important: This checklist is for first-time applicants only. If you have ever held a teaching license in Colorado, this is the wrong checklist. Instead, find your license type below and follow the link to the appropriate information for you:
Plan ahead: In some
cases you may need to request transcripts or other documentation from third parties to complete the online application.
Allow yourself plenty of time to obtain these documents before your application is due.
Please note that you will need electronic copies of all documentation listed below so you can upload them to CDE as you complete your online
application.

Send fingerprint card to CBI.
Note: You only have to be fingerprinted one time for CDE.
Name changes only: Obtain a copy of
marriage license, divorce decree, or other legal documentation.

Document employment history for the past 5 years.
Include employer/business name, dates of employment, reason for leaving, work addresses, supervisor names, and contact information.
Include teaching experience from more than 5 years ago, if you have it.
If applicable, obtain supporting documentation related to past criminal history, disciplinary actions, or any issues related to licensure or employment.

List the names and addresses of the
colleges/universities you've attended.
Obtain copies of your official transcripts indicating that you have earned a minimum of a Bachelor's Degree.
Note: Include only transcripts from colleges and universities.

Download and complete the Approved Program Verification Form.
Out-of-state alternative applicants: Obtain an official signed letter from your State Department of Education confirming that you completed a state approved program and the program requirements for the alternative program you completed.
Obtain the results for any CDE-accepted PLACE or PRAXIS II exams you have passed.

Once you have gathered all of the information listed above, you are ready to start your online application!
